Mounjaro is the brand name for tirzepatide, a once-weekly injectable medicine developed by Eli Lilly. It acts on two gut-hormone receptors at the same time, GIP and GLP-1. That dual action reduces appetite, slows stomach emptying and improves how the body handles blood sugar.

In South Africa, Mounjaro is distributed by Aspen Pharmacare in partnership with Lilly. It is a Schedule 4 medicine, so it always requires a doctor's assessment and authorisation, and it is dispensed by licensed pharmacies only.

Is Mounjaro approved by SAHPRA?

Yes. Mounjaro is fully registered with SAHPRA. The timeline matters because the two indications arrived separately.

Aspen launched Mounjaro in South Africa in December 2024 for adults with type 2 diabetes. In October 2025, SAHPRA approved tirzepatide for chronic weight management: as an addition to a reduced-calorie diet and increased physical activity in adults with obesity, or who are overweight with at least one weight-related condition.

The registered use is alongside diet and activity changes, not instead of them. That is also how the clinical trials that support the registration were run.

Registration also settled the presentation. The multi-dose KwikPen has replaced the single-dose vials Aspen launched with, so the pen is what South African pharmacies now stock.

Who is Mounjaro registered for in South Africa?

For weight management, the SAHPRA-registered use covers adults with a BMI of 30 or more, or a BMI of 27 or more with at least one weight-related condition. Common qualifying conditions include high blood pressure, high cholesterol, type 2 diabetes, obstructive sleep apnoea and cardiovascular disease.

BMI is a screening number, not a full picture. A doctor also weighs up your medical history, current medicines, previous weight loss attempts and any contraindications before deciding whether treatment is clinically appropriate.

Mounjaro is not registered for cosmetic weight loss, for people in a healthy weight range, or for anyone under 18. South Africa receives the Mounjaro KwikPen, a pre-filled multi-dose pen. Each pen contains four weekly doses. Treatment starts low and steps up over several months so the body can adjust.

Your doctor decides the pace of increases based on how you respond. Not everyone needs the maximum dose; many people settle at a middle dose that balances results and side effects.

How do you store Mounjaro in South Africa?

Unopened Mounjaro pens live in the fridge at 2 to 8 degrees Celsius. The medicine must never freeze: a pen that has frozen, even once, must not be used, because freezing damages the active ingredient in ways you cannot see or taste.

Once you take the first dose, the rules loosen. Lilly states the in-use KwikPen may be kept below 30 degrees Celsius for up to 30 days, and that moving it between fridge and room temperature within that window does not harm the medicine. After 30 days, or after the fourth weekly dose, the pen is discarded. Never store the pen with a needle attached, because that can draw in air bubbles that affect dose accuracy.

Load shedding threatens the fridge less than most people assume. A closed fridge holds a safe temperature for a few hours, so a two-hour outage needs nothing more than a shut door. For longer outages, move pens to a cooler box with ice packs, wrapped in a cloth so the pen never rests directly against the packs and cannot freeze. Can you travel with Mounjaro pens?

Yes, and the 30-day in-use allowance below 30 degrees Celsius makes most trips straightforward. Only two things ruin a pen in transit: freezing, and heat above 30 degrees. Plan around those and the rest is admin.

On flights, pens go in hand luggage, never in checked baggage. The cargo hold can drop below freezing, and a frozen pen must be discarded. Airport scanners do not harm the medicine. How well does Mounjaro work for weight loss?

The key evidence is SURMOUNT-1, a 72-week trial of 2,539 adults with obesity or overweight, published in the New England Journal of Medicine. Average weight loss was 15% of body weight on 5 mg, 19.5% on 10 mg and 20.9% on 15 mg, compared with about 3% on placebo. Individual results vary.

The responder rates tell the same story. Roughly nine in ten participants on the two higher doses lost at least 5% of their body weight, compared with about a third of the placebo group. Individual results vary.

Trial participants also followed a reduced-calorie diet and an activity plan. The medicine supports those changes; it does not replace them.

What are the side effects of Mounjaro?

The most common side effects are digestive: nausea, diarrhoea, constipation, vomiting and indigestion. They are usually mild to moderate, appear most often when the dose increases, and tend to settle as the body adjusts. Slower dose increases, smaller meals and good hydration all reduce the impact.

Mounjaro is not suitable for everyone. It should not be used in pregnancy or while breastfeeding, or by people with a personal or family history of medullary thyroid cancer or MEN2 syndrome. A history of pancreatitis needs careful discussion with a doctor. Severe, persistent stomach pain can signal pancreatitis: seek medical care immediately.

Published South African pharmacy prices in 2026 range from roughly R3,600 to R8,200 per KwikPen depending on the strength. One pen holds four weekly doses, so a pen covers about a month. Can you get Mounjaro at a government hospital or clinic?

No. Mounjaro is not stocked in South Africa's public sector, and neither is any other medicine in this class. Access in 2026 runs through private doctors and private pharmacies only, which is why cost dominates so much of the conversation.

That may not be permanent. Spotlight reported in February 2026 that the National Essential Medicines List Committee is prioritising a review of GLP-1 medicines for the public sector list, though no timeline has been committed. The World Health Organization added tirzepatide and three GLP-1 medicines to its Model List of Essential Medicines in September 2025, and a WHO listing typically strengthens the case for national adoption.

Yes. Switching to Mounjaro from another injectable treatment is common, and safe when a doctor manages it. The usual reasons are a results plateau, side effects that will not settle on the current medicine, or persistent stock problems with a specific product.

Two rules shape every switch. First, doses do not transfer: tirzepatide and semaglutide are different molecules with different dosing ladders, so everyone starts Mounjaro at 2.5 mg weekly regardless of the dose they were on before.
