Mounjaro vial vs pen: what is the difference?

What are the two Mounjaro formats?

The Mounjaro KwikPen is a pre-filled, single-patient pen containing four fixed doses of one strength. You screw on a fresh pen needle, prime the pen, dial one dose and inject. After four weekly injections the pen is empty and is discarded.

The Mounjaro vial is a small glass container holding one 0.5 ml dose. You draw the full contents into a separate syringe, inject, and discard the vial, even if a little liquid remains. Nothing about it is reusable.

Two formats exist because of manufacturing, not medicine. Vials are quicker to produce at scale than pen devices, so when global demand for tirzepatide surged, vials let Lilly reach more markets sooner while pen capacity caught up. South Africa's vial-first launch fits that pattern.

Both formats deliver the same subcutaneous injection into the stomach, thigh or back of the upper arm, and both require a sharps container for used needles.

Which format do you get in South Africa?

Today, mostly the KwikPen. Aspen, which distributes Mounjaro locally in partnership with Lilly, launched the medicine in single-dose vials in late December 2024 while the KwikPen was still under evaluation at SAHPRA. Once the pen was registered and launched in 2025, pharmacy supply moved to it.

Dischem, Clicks and independent pharmacies now stock the pen across the strength range, subject to the stock fluctuations that affect this whole class of medicine. Vials may still surface in the supply chain, which is why it is worth knowing both formats exist.

Stock levels shift week to week, and individual strengths sell out at branch level. Phone your branch before collection day, and if your strength is out everywhere nearby, ask your doctor before accepting a substitute; changing strength or format is a treatment decision, not a till decision.

How does the handling differ day to day?

The pen is built for self-injection. The dose is fixed, the window confirms delivery, and the routine is the same every week: needle on, prime, dial, inject, count to five. Most people manage it confidently after one demonstration. The full routine is in how to inject Mounjaro.

The vial asks more of you. You draw the dose into a syringe yourself, which means checking the liquid, drawing up the full contents, clearing air bubbles from the syringe, and injecting with a steady hand. It is the same skill diabetics used for decades, and it is entirely learnable, but there are more points where technique matters.

For both formats the first demonstration should come from a healthcare professional, and the injection itself is identical: under the skin, rotating sites each week.

Eyesight and dexterity are worth raising at that demonstration. The official pen instructions state that people with impaired vision should not use the KwikPen without help from someone trained on it, and stiff or painful hands can make the vial's draw-up step harder than the pen's dial. If either applies to you, a family member can be trained to help, and the back of the upper arm becomes a usable site when someone else injects.

Do the storage rules differ?

Both formats live in the fridge at 2 to 8 degrees Celsius until use, must never be frozen, and should be inspected before injection: the liquid must be colourless to slightly yellow, with no cloudiness or particles.

The pen's rule kicks in at first use: once you have injected from it, it may stay out of the fridge below 30 degrees Celsius and must be finished or discarded within 30 days, which matches its four weekly doses.

The vial's allowance is cumulative: the South African patient leaflet permits up to 21 days in total below 30 degrees Celsius, and an opened vial is discarded straight after its single use regardless.

In practice both travel well for short trips with a cooler bag. Neither should ever wait in a hot parked car.

Is the Mounjaro vial cheaper than the pen?

Per week, the published prices have been broadly similar at equivalent strengths. At the December 2024 launch, vials were reported at about R880 per weekly dose for 2.5 mg and 5 mg, and about R1,140 for 7.5 mg and 10 mg. Published 2026 KwikPen prices run from roughly R3,606 to R8,168 per pen, and each pen holds four weekly doses. Prices vary by pharmacy and dose; confirm with your pharmacy. Divide the pen price by four and the lower strengths land near R900 per week, in the same territory as the launch vials. The pen's costs are simply paid a month at a time.

Does the format change the dose or the results?

No. Vial or pen, the medicine is tirzepatide, the dose schedule is the same, and the clinical evidence applies equally. Format is about convenience and handling, not effectiveness.

The side effects are the same too. The common ones are digestive: nausea, diarrhoea, constipation and vomiting, mostly around dose increases, usually settling as the body adjusts. Mounjaro is not suitable in pregnancy or while breastfeeding, or for people with a personal or family history of medullary thyroid cancer or MEN2 syndrome. Severe, persistent stomach pain can signal pancreatitis: seek medical care immediately.

One caution belongs here. Because vials and pens contain the same liquid, some people are tempted to stretch supply by splitting pen doses or pooling vials. Do not. The pen delivers fixed doses by design, an opened vial is not sterile after use, and any dose change belongs to your doctor, who sets and adjusts your dose based on your response.
